If you live abroad, you can order one from your local GP surgery. You can request to be made a resident or a non-resident on the HC1 form (the form will take up to 10 working days to process). You must also be over 25. If you want to live at a different address in the UK, you will normally have to be referred for a residence (and/or health care) order from an external UK body (for example the DSP). If you don't have any of these things already, and you have been here for a year or more and haven't made any applications to change your UK status, you may be eligible to apply.
